Marketed Inflammatory Bowel Disease Therapies

  • RINVOQ (upadacitinib): AbbVie RINVOQ is a Janus kinase (JAK) inhibitor. JAKs are intracellular enzymes that transmit signals arising from cytokine or growth factor-receptor interactions on the cellular membrane to influence cellular processes of hematopoiesis and immune cell function. Within the signaling pathway, JAKs phosphorylate and activate signal transducers and activators of transcription (STATs) which modulate intracellular activity including gene expression. Upadacitinib modulates the signaling pathway at the point of JAKs, preventing the phosphorylation and activation of STATs. In March 2022, the US FDA approved RINVOQ for the treatment of adults with moderately to severely active ulcerative colitis.
  • STELARA (ustekinumab): Janssen Pharmaceuticals STELARA is a human monoclonal antibody. It mediates the body’s T-cell response by acting as an antagonist against interleukin-12 (IL12) and interleukin-23 (IL23). IL-12 and IL-23 are cytokines that modulate lymphocyte function and have been implicated in the pathogenesis of inflammatory diseases. In September 2016, the US FDA approved STELARA for the treatment of adults with moderately to severely active Crohn’s disease.

Emerging Inflammatory Bowel Disease Therapies

  • ABX464 (obefazimod): Abivax ABX464 is an oral small-molecule drug candidate developed by Abivax with potent anti-inflammatory properties. It selectively upregulates microRNA-124 (miR-124), a natural regulator of the inflammatory response, which leads to decreased cytokine production and immune cell activity, helping to reduce inflammation. Currently, it is being evaluated in various Phase III trials to treat moderately to severely active Ulcerative Colitis, with topline data readout expected in late Q2, 2026. For Crohn’s disease, the asset is currently being evaluated in a Phase IIb induction study, with topline results anticipated in Q4 2026. In January 2026, Abivax announced that ABTECT-UC Phase III maintenance topline results for ABX464 are expected in late Q2 2026, followed by a planned US regulatory filing in late 2026.
  • RHB-204: RedHill Biopharma RHB-204 is an investigational, proprietary, fixed-dose, orally administered antibiotic combination therapy containing clarithromycin, rifabutin, and clofazimine in development for the treatment of moderate to severe Crohn’s disease in Mycobacterium avium subspecies paratuberculosis-positive (MAP+) patients. RHB-204 is currently being evaluated in a planned innovative Phase II clinical study.
